New Patient Registration and Change of Doctor

Legal basis: Performance of a contract (Art. 6(1)(b) GDPR) – processing is necessary to take steps prior to entering into an agreement for the provision of healthcare services.

Provision of Healthcare Services

Legal basis: Legal obligation (Art. 6(1)(c) GDPR) in conjunction with Art. 9(2)(h) GDPR – processing is necessary for the purposes of preventive or occupational medicine, assessment of the working capacity of an employee, medical diagnosis, the provision of health or social care, or treatment.

Requesting Medical Records from Previous Doctor

Legal basis: Legal obligation under Act No. 576/2004 Coll. on Healthcare.

Contacting the Patient

Legal basis: Legitimate interest of the controller (Art. 6(1)(f) GDPR) – contacting you to arrange an appointment and provide information related to registration.

Processing of Special Categories of Personal Data

The birth number and health-related data constitute special categories of personal data. Their processing is necessary for the provision of healthcare services and is based on:

Act No. 576/2004 Coll. on Healthcare and Healthcare-Related Services.

Act No. 153/2013 Coll. on the National Health Information System.

Article 9(2)(h) GDPR – processing for the purposes of preventive or occupational medicine, assessment of the working capacity of an employee, medical diagnosis, the provision of health or social care, or treatment.

Recipients of Personal Data

Your personal data may be disclosed to the following recipients:

Health insurance companies – Dôvera, Union, Všeobecná zdravotná poisťovňa – for reporting healthcare services and reimbursement purposes.

Previous general practitioner – for requesting and transferring medical records in accordance with the law.

National Health Information Centre – in accordance with Act No. 153/2013 Coll. on the National Health Information System.

Other healthcare providers – when referral to a specialist is needed or to ensure continuity of healthcare.

Public authorities – in cases required by law (e.g., Public Health Authority of the Slovak Republic).

We do not transfer your personal data to third countries outside the European Economic Area.

Retention Period for Personal Data

We retain your personal data for the following periods:

Data from the patient registration form – until the completion of the patient registration process, after which it becomes part of the medical records. If registration does not take place, we will delete the data within 30 days.

Medical records – 20 years from the last provision of healthcare, in accordance with Section 22 of Act No. 576/2004 Coll. on Healthcare.

Accounting documents – 10 years in accordance with accounting and tax regulations.

After the retention period expires, your personal data will be securely deleted or anonymised.

Your Rights

In connection with the processing of your personal data, you have the following rights:

Right of Access

You have the right to obtain confirmation as to whether we process your personal data, and if so, you have the right to access such data and information about its processing.

Right to Rectification

You have the right to request the correction of inaccurate personal data or the completion of incomplete personal data concerning you.

Right to Erasure

You have the right to request the deletion of your personal data if it is no longer necessary for the purposes for which it was collected, or if you withdraw consent and there is no other legal basis for processing. This right is limited in the case of medical records, which we are legally required to retain for the period specified by law.

Right to Restriction of Processing

You have the right to request restriction of processing of your personal data in certain cases, for example, if you contest the accuracy of the personal data.

Right to Data Portability

You have the right to receive the personal data concerning you, which you have provided to us, in a structured, commonly used, and machine-readable format.
